GENERAL PROCDURE FOR LOKMANYA HOSPITALS
INPATIENTS

PATIENT GUIDELINES
  • We provide care with dignity, consideration and respect for privacy / confidentiality.
  • The bed charges are calculated from 12.00 am to 12.00 am irrespective of the time of admission.
  • If you have a mediclaim policy, you are required to approach the Company directly for reimbursement of your bills for which you will need (i) the final bill, (ii) the detail bill, (iii) the discharge card and (iv) narrative summary. The final and detail bill will be given by cashier on settlement of dues. The discharge card will be given by the ward nurse at the time of discharge. ( For Help contact Billing Department)
  • Charges for procedures will be billed according to the class of accommodation.
  • If you are admitted for an Emergency or Special operation, the following deposits will be collected as booking charges -
  • In case you are a non - appointment / walk-in patient, you may have to wait to see the doctor.
  • Rs. 1000 /- to Rs. 2000 /- for Operation Theater
  • Rs. 1,500 /- for Angiography
  • Rs. 10,000. /- for Open Heart Surgery and Angioplasty
  • No refunds
  • Please Note transfer to lower class will be considered only after one day after the request is made.
  • Transfer to higher class will mean paying all higher-class charges from the day of admission, and not from the date of transfer.
  • When a patient is transferred to the ICU, the occupied room is to be vacated. However in case the patient's relation desires to retain the room, the charges will be 1½ times of the normal charges. This facility will be provided only if room is available, In case of emergency, the retained room will have to be vacated immediately
  • To help you keep track, provisional bill will be submitted to you once in 2 days for I.C.U., 4 days for ordinary bed.
  • In case you have to leave suddenly, a suitable deposit will be collected by the billing office. This amount will be adjusted against the final bill.
  • If the person who had signed the authorization / consent form cannot be present when the remaining deposit is to be collected, he should issue a letter of authority to the person who will do so.
  • Final Bill will be issued in original on the day of discharge
  • Receipts of all payments will be issue as soon as the bill is settled.
  • Notification of pass cancellation is must with the security,
  • Our pharmacy provides all the medicines for inpatients. We do not permit medicines & consumables from outside. Charity provision is made available for economically unaffording patients.
  • Unused drugs supplied by the Hospital pharmacy may be returned even if partially used provided the batch number and expiry date are clearly visible. Refund amount at the issue price will be adjusted in the bill as replacement.
  • The attendant's pass of Rs. 200/- is valid for six days. This allows one person to stay with the patient. Please return this pass when you leave, it is necessary for the security purpose.
  • One additional visitor’s pass (07.00 am - 07.00 pm) per patient is issued by the security department (Valid for three days) on prescribed form available with nursing staff in each ward.
  • Kindly bring the original pass issued during admission, for renewal of additional pass.
  • For patients, food will be served from the hospital kitchen only. For relatives/attendants meals will not be served by the hospital
  • 24 hours Ambulance service is available on payment. For booking please contact the admission counter in advance.
  • 15% discount is available to all Senior Citizens card holders for OPD consultations/procedures/test.
  • There is no surcharge on the hospital bill, however all foreign nationals and NRIs will be levied 25% surcharge on the gross bill.
  • We do not encourage patient's relatives to stay overnight with patient's in shared rooms/wards.
ADMISSION PROCEDURE
Ways of Admission
  • Through the admission counter, if you are referred to our hospital by your family doctor, and if urgent medical attention is not required.
  • Through the Casualty Medical Officer in the Casualty Department, if you are either -
  • In urgent need of medical attention, or
  • You come without being referred and therefore need to be admitted under the appropriate consultant doctor
For Admission – Reception Counter
Registration Form
  • If you are a new patient wishing to consult any of our specialists for an outpatient, you are required to fill up this registration form.
  • This form is available at our registration counter. One can download the form from this site, fill it up and get a printout and can come directly to the Registration / Reception Counter. (Please click on the links below to download the form and please enter your details correctly.)
  • Individuals who have an LH Registration number, by virtue of being our patient previously at our OPD/Health Check up/Casualty/Emergency are not required to fill this form.
  • Kindly quote your unique LH Registration number directly at the counter for Registration.

Submit details of your case i.e. either –

  • Your doctor's reference note [which will contain instructions]
  • Your Company ID Card or Employee Authorization Letter
  • Medical Insurance Policy ( Xerox Copy)
  • The Casualty Medical Officer's note.
Fill in the following documents –
  • Admission Form - This is to ensure you receive appropriate medical treatment while in our care
  • Consent Form - relating to your treatment, this form is important; it seeks your acceptance. [Either you, your relative or a friend should sign your consent form.]
  • All patients for admission are requested to sign a declaration for payment as per the hospital charges. Bills will need to be settled by the person who has signed this.
  • Select the class in which you would like to be admitted. [Please remember that if you desire a change, immediate transfer to another class may not be possible.]
DISCHARGE PROCEDURE
  • Patient Discharge time is 11.00 a.m. [In case the Patient discharge formalities are completed before 10.00 a.m. and the bills are settled on the same day of your discharge, the patient may leave by 12 a.m.]
The procedure for discharge is as follows –
  • The Consultant doctor will make an entry of discharge in your case folder.
  • The Resident doctor will then fill in your discharge card with details and the further course of treatment. If you need to come back for a follow-up assessment, it will be indicated. Drug prescriptions and other instructions will also be given.
  • Your folder containing the relevant papers and vouchers will be sent to the Billing Department.
  • Admission counter will be informed about your discharge and the social worker and housekeeping department in-charge shall make a visit for other formalities.
  • The final bill is readied after the deposit is taken into account. When you receive the final bill, payment has to be made at the Cash counter in the podium.
  • A security deposit of Rs. 5000/- is retained. It may be collected after a week.
  • Pending reports are to be collected from the centralized reports counter between 8.00 am to 8.00 pm, Monday through Saturday.
